Vascular surgery in India
Vascular surgery treats the arteries and veins outside the heart and brain: aneurysms that can burst without warning, blocked leg arteries that threaten a limb, and varicose veins that are usually a quality-of-life problem rather than a danger. The three could not be more different in urgency.
- Commonly treated
- Aortic aneurysm, blocked leg arteries, varicose veins, diabetic foot
- Indicative cost
- Aneurysm repair $8,500; TEVAR stent graft $22,000; stenting $15,000
- Two approaches
- Open surgery, or endovascular repair from inside the vessel
- Most urgent
- A large or enlarging aneurysm, and a limb that is cold, pale and painful
- Least urgent
- Varicose veins, which are rarely dangerous and often cosmetic
- First step
- A vascular surgeon reviews your CT or ultrasound on video, free
What cannot wait
Go to an emergency department where you are, now, if: you have sudden severe pain in the abdomen or back with a known aneurysm; or a leg or foot that has become suddenly cold, pale, numb and painful. The first can be an aneurysm leaking, the second a blocked artery starving the limb, and both are measured in hours. Do not arrange international travel around either.
Aortic aneurysm
The aorta is the main artery leaving the heart. An aneurysm is a weakened section that balloons outwards, and the danger is rupture, which is frequently fatal. It usually causes no symptoms at all, which is why most are found incidentally on a scan done for something else.
The decision to repair rests on size and on how fast it is growing, weighed against the risk of the operation itself. A small, stable aneurysm is monitored with scans and blood pressure control, and operating on it early does not help. Above a threshold, or when it is enlarging steadily, repair becomes the safer course. That threshold is a judgement your surgeon makes from your scans, your blood pressure and your general health.
Two ways to repair it:
- Open repair. The weakened section is replaced with a graft through an incision. A bigger operation with a longer recovery, and a very durable result. An indicative $8,500.
- Endovascular repair. A stent graft is delivered from inside the vessel, usually through the groin arteries, and lines the aneurysm from within. Far less invasive with a much shorter recovery, at an indicative $22,000 for TEVAR with a five-day stay. The trade-off is that it requires suitable anatomy and lifelong surveillance scans, because the graft can develop leaks over time that need attention.
That surveillance requirement matters particularly if you are travelling from somewhere with limited access to CT scanning. An endovascular repair that is never scanned again is not the lower-risk option it appears to be. Raise where you live, honestly, when the two are being compared for you.
Blocked leg arteries
The same fatty narrowing that affects heart arteries affects leg arteries. Early on it causes cramping pain in the calf on walking that eases with rest. Later it causes pain at rest, particularly at night, and eventually ulcers or gangrene that threaten the limb.
Treatment ranges from supervised exercise and medication in mild cases, through angioplasty and stenting from inside the artery, to a bypass graft around the blockage. Endovascular stenting is priced at an indicative $15,000 with a six-day stay.
The honest framing: the earlier stages are managed rather than cured, and stopping smoking does more for the outcome than any procedure. The later stages are about saving a limb, and then speed matters.
The diabetic foot
This deserves its own section because it is common in our markets and because amputations that follow it are frequently preventable.
Diabetes damages both the nerves and the arteries of the foot. Damaged nerves mean an injury is not felt; damaged arteries mean it does not heal. A small unnoticed wound becomes an ulcer, the ulcer becomes infected, and the infection reaches bone. By the time it hurts enough to prompt a hospital visit, options have narrowed considerably.
Limb salvage is a genuine specialty combining vascular surgery to restore blood flow, careful debridement, infection control, and offloading the pressure on the wound. If you have been told an amputation is needed, a vascular opinion on whether blood flow can be restored first is worth seeking before agreeing. Sometimes it can, and sometimes it cannot, but it is the right question to have asked. See also diabetes and endocrine care.
Varicose veins
The opposite end of the urgency scale. Varicose veins are enlarged surface veins caused by failing valves, and for most people they are an appearance and comfort problem rather than a medical danger. Aching, heaviness and swelling after long standing are typical.
They do need attention when they cause skin changes around the ankle, a healed or open ulcer, or repeated inflammation, because those indicate sustained pressure that will keep causing damage. Treatment is now usually done without surgery: laser or radiofrequency ablation closes the faulty vein from the inside, done as a day case with a quick return to normal activity.
Varicose vein treatment is not on the partner price list, so there is no figure for it here. Be aware also that recurrence over the years is common with every technique, which is worth knowing before travelling specifically for it.
| Procedure | Indicative cost | Hospital stay |
|---|---|---|
| Aortic aneurysm repair | $8,500 | Extended |
| Endovascular stenting | $15,000 | 6 days |
| TEVAR (stent graft for the aorta) | $22,000 | 5 days |
Questions patients ask
How much does aortic aneurysm surgery cost in India?
The indicative partner-hospital price is $8,500 for open aneurysm repair and $22,000 for TEVAR, the stent graft approach, which carries a five-day stay. Endovascular stenting is $15,000 with a six-day stay. The device accounts for most of the difference between open and endovascular repair.
Should I have open or endovascular aneurysm repair?
Endovascular repair is far less invasive with a much shorter recovery, but it needs suitable anatomy and commits you to lifelong surveillance scans, because stent grafts can develop leaks. Open repair is a bigger operation with a longer recovery and a very durable result. If CT scanning is hard to access where you live, say so, because it genuinely affects which is safer for you.
Does my aneurysm need operating on now?
It depends on its size and how fast it is growing, weighed against the risk of the operation. Small, stable aneurysms are monitored with scans and blood pressure control, and operating early does not help. A surgeon reading your CT can tell you which side of that line you are on.
I have been told I need an amputation. Is there an alternative?
It is worth getting a vascular opinion on whether blood flow to the limb can be restored first, because limb salvage combines restoring circulation with debridement, infection control and offloading the wound. Sometimes the limb can be saved and sometimes it cannot, but it is the right question to have asked before agreeing.
How much does varicose vein treatment cost in India?
It is not on the partner price list we publish from, so we quote no figure. You would receive a written estimate after a consultation. Be aware that recurrence over the years is common with every technique, which is worth knowing before travelling specifically for it.
Are varicose veins dangerous?
Usually not. For most people they are a comfort and appearance problem. They do need attention when they cause skin changes around the ankle, an ulcer, or repeated inflammation, because those indicate sustained pressure that will keep causing damage.
My leg has suddenly gone cold and pale. What should I do?
Go to an emergency department where you are, immediately. A suddenly cold, pale, numb and painful limb can mean an artery has blocked, and it is measured in hours. Do not plan international travel around it. Deal with it locally first.
