Cosmetic and reconstructive are not the same

Both are done by plastic surgeons and the techniques overlap, but the reasons differ and so does the way you should think about them.

Reconstructive surgery restores form and function after cancer, burns, injury or a condition present from birth: breast reconstruction after a mastectomy, cleft lip and palate repair, scar and burn contracture release, nerve and tendon repair after trauma. This is medical need, and for many patients from our markets it is the reason for the trip. Cosmetic surgery changes appearance where there is no functional problem. It is entirely legitimate, and it deserves more scepticism at the point of sale, because the person selling it benefits from your saying yes and nothing is medically lost if you say no.

How to check the surgeon, specifically

This matters more here than in almost any other specialty on this site, because cosmetic procedures are performed in many countries by practitioners without plastic surgery training. Three concrete checks:

Ask for the qualification by name. A plastic surgeon in India holds a recognised postgraduate surgical qualification in plastic surgery, typically an MCh or DNB following a general surgery degree. Ask which one, and from where. A doctor with a different specialty and a short course in a technique is not the same thing.
Ask how many of your specific procedure they do a year. Not how many operations in total. Volume in the exact procedure is the thing that correlates with results.
Ask to see their own before-and-after photographs, of patients with a starting point like yours, taken at a consistent distance and lighting. Generic stock galleries tell you nothing.

A good surgeon will also tell you what they cannot achieve. If everything you ask for is met with yes, that is a warning rather than reassurance. Realistic limits, stated before surgery, are the clearest signal of a surgeon worth travelling to.

The mistake that causes most problems: leaving too early

Cosmetic surgery abroad has a poor reputation in some countries, and when you examine why, the pattern is rarely bad surgery. It is patients booking a flight home three or four days after an operation, developing a complication once they land, and having nobody to manage it.

Wound infections, collections of fluid, and problems with healing typically declare themselves between roughly day five and day fourteen. That is precisely when a lot of medical tourists are already home and their surgeon is several thousand kilometres away. Body-contouring surgery in particular carries a raised risk of blood clots, and a long flight too soon compounds it.

Two rules that prevent most of this. Stay in India until your surgeon has reviewed the wounds and cleared you to fly, not until your ticket says so. And before you travel, identify a surgeon at home willing to see you if something goes wrong, because "I had it done abroad" is a difficult conversation to start in an emergency department.

Revision, and who pays for it

Some cosmetic operations need a second procedure, either because healing was unpredictable or because the result did not match what you wanted. This is normal in the field and it should be discussed before the first operation, not after.

Ask directly: if a revision is needed, what would it cost, and would I have to travel back? Get the answer in writing. A revision that requires a second international trip is a real cost and belongs in your budget from the start.

Being fit for surgery

Elective cosmetic surgery is worth doing only from a reasonable baseline of health, which is why a proper consultation asks about things that seem unrelated. Smoking meaningfully impairs wound healing and most surgeons will ask you to stop well before surgery. Uncontrolled diabetes raises infection risk. Significant obesity raises anaesthetic and clot risk, and for body contouring a stable weight gives a far better and longer-lasting result than operating during active weight change.
