Some of this work needs an operation; a growing part of it does not, because radiosurgery can treat targets inside the head without an incision at all.

What is treated
Brain tumours
Not all are cancer. Surgery removes what can safely be removed, and where a tumour sits near critical structures, radiosurgery may treat it without an incision.
Movement disorders
Deep brain stimulation for Parkinson's disease, essential tremor and dystonia. Electrodes placed precisely in the brain and driven by an implanted pulse generator.
Epilepsy surgery
For seizures that continue despite properly trialled medication. Only worthwhile once the seizures have been localised to one place that can safely be removed.
Radiosurgery
Gamma Knife and CyberKnife deliver focused radiation to a target inside the head with no incision, usually in a single session, often as a day case.
Open surgery or radiosurgery
This is the question that decides most neurosurgical plans, and it is worth understanding before anyone quotes you for either.
Open surgery, a craniotomy, physically removes tissue. It is the right answer when a tumour is large, when pressure inside the head needs relieving now, or when a piece of tissue is needed for a diagnosis. Radiosurgery does not remove anything: it delivers a precisely shaped dose of radiation that stops the target growing and, over months to years, often shrinks it. It suits smaller, well-defined targets, particularly those sitting close to structures a surgeon could not reach without cost, and it avoids a general anaesthetic and a hospital stay almost entirely.
A great deal depends on the border of the target. Sharply defined lesions such as many meningiomas, acoustic neuromas, brain metastases and arteriovenous malformations are radiosurgery's natural territory. Deep brain stimulation for Parkinson's
Deep brain stimulation places fine electrodes in a specific target within the brain, connected to a pulse generator implanted under the skin of the chest. The stimulation is adjustable and reversible, which is its great advantage over older operations that destroyed tissue permanently.
Who it helps: people whose Parkinson's still responds well to levodopa but whose response now fluctuates through the day, with periods when the medication has worn off, or who have troubling involuntary movements caused by the medication itself. It typically improves tremor, stiffness and those fluctuations.
What it does not do is worth stating just as clearly. Deep brain stimulation does not cure Parkinson's disease and does not stop it progressing. Symptoms that do not respond to levodopa generally do not respond to stimulation either, and problems with balance, speech and memory usually fall into that group. A neurologist assessing you should be as interested in what will not improve as in what will.
On price: at an indicative $27,000 to $28,000 for a non-rechargeable system and $32,000 for a rechargeable one, this is one of the most expensive procedures on our site, and the reason is the device rather than the surgery. A rechargeable generator costs more initially but lasts far longer before it needs replacing, and replacement means another operation. If you live far from a centre that can change a generator, that arithmetic matters more than the initial price.
Epilepsy surgery
Epilepsy surgery is considered when seizures continue despite two or more anti-seizure medicines that have been properly trialled at adequate doses. At that point further drug changes are much less likely to work, and continuing to try them for years has its own cost.
The operation itself is the last step of a long assessment. Video EEG monitoring, detailed MRI, often functional imaging and neuropsychological testing are used to establish that the seizures come from one identifiable place, and that removing it will not take something you cannot afford to lose, such as language or memory. Where that assessment lines up, surgery offers a real prospect of seizure freedom. Where it does not, the honest answer is that surgery is not the right route, and the assessment is what tells you which of the two you are.

Time in India and flying home
Most cranial surgery involves four to five days in hospital, but the trip is longer than that because the assessment comes first and because flying after intracranial surgery needs clearance. Plan on three to four weeks for a craniotomy. Deep brain stimulation adds programming sessions after the implant, when the stimulation settings are tuned to your symptoms over several visits, so that timeline is longer again and should be agreed in advance.
Radiosurgery is the exception in every respect: typically a single session, often as a day case, with a much shorter trip. If your case is suitable for it, that difference in time and cost is substantial.

Can my brain tumour be treated without opening the skull?
Sometimes. Radiosurgery with Gamma Knife or CyberKnife treats sharply defined targets such as many meningiomas, acoustic neuromas and brain metastases in a single session with no incision. Large tumours, tumours causing pressure that must be relieved now, and diffuse tumours without a clear border generally need surgery. Your MRI decides which applies.
